V-Tach or Wide Complex Tachycardia with Pulse

Algorithm
 

V-Tach is defined as heart rate > 100 regular, QRS > .12sec

 

Stable V-Tach: (Systolic BP > 90, good mental status, minimal chest discomfort):


- Monitor vital signs, ALS escort to nearest ERC

 

Unstable V-Tach: (Systolic BP ≤ 90, ALOC, chest pain, or signs of poor perfusion):


Cardioversion:

Philips (100j)

Zoll (70j)

Lifepack (
(do not delay for IV access if deteriorating);

Versed (Midazolam) - BHO

 

→ If cardioversion is unsuccessful:

Amiodarone 150 mg slow IV (over 1 minute)


Allow Amiodarone to circulate for 2 minutes


Cardioversion:

Philips (200j)

Zoll (120j)

Lifepack (


Amiodarone 150 mg slow IV (over 1 minute)


Allow Amiodarone to circulate for 2 minutes


Cardioversion:

Philips (200j)

BHO:

For further orders, or
AICD firing > 2 times within 15 min.
